On an autumn day several years ago, we lost one of our daughters. Her name was Laura Anne. She was a precious little five-month-old girl, and she had Down Syndrome. Unfortunately, she had a heart defect and didn’t pull through heart surgery. Predictably, losing her was one of the toughest times of our lives. We miss her tremendously and always will; yet we take comfort knowing we’ll see her again in heaven.

Emdogain (EMD) is a purified extract of developing embryonal enamel derived from 6-month old piglets that we use in our surgical procedures to enhance regeneration. In short, EMD helps stimulate regeneration of the bone defects that occur from periodontal inflammation.
In the May 2017 Journal of Periodontology, there was an article about a study that showed that EMD alone could stimulate bone growth and regeneration as well as EMD with bone regeneration. The study also reveals a regeneration potential that allows bone healing by blocking epithelial down growth between the graft and root and stimulating regeneration.
EMD alone has been shown to aid in healing of soft tissue graft procedures allowing more potential for root coverage where deemed necessary. Here at North Raleigh Perio, we have seen success in incorporating this regenerative technology.

As a periodontist here in Raleigh, I have had the opportunity to perform implants for the past 31 years. Dental implants are often considered a viable option by patients with missing teeth due to injury, decay, periodontal issues, or congenital deficiencies. In placing implants, I have seen both immediate and delayed dental implant options provide aesthetic and practical solutions to patients.
There are pros and cons to both implant types including aesthetic and overall health considerations. The success rate appears to be slightly higher with delayed implant treatment according to my experience and the European Journal of Oral Implantology. It has been a busy beginning of 2017. We started off with the North Carolina Society of Periodontists meeting in Raleigh with Dr. Greggory Kinzer from Seattle Washington lecturing on an Interdisciplinary Approach to Managing Esthetic Dilemmas. His philosophy was delayed treatment when possible if certain paradigms were satisfied.
Let’s talk about inflammatory gum disease treatment. It really is quite simple: remove the bacteria. There are two types of bacteria that contribute to inflammatory gum disease: plaque (the live bacteria) and calculus (the dead bacteria).
The physical removal of the live bacteria is done initially by scaling and root planing. Some operators use local antibiotic therapy as a treatment addition. The additional treatment adds costs and, according to an article in the October 2016 issue of The Journal of Periodontology, has no additional benefit.
The calculus, otherwise known as tarter, needs to be physically removed. If pockets are greater than 5mm, we cannot remove it effectively with root planing . It will need to be visualized by a flap procedure to see it. Unfortunately, you can not laser calculus off the teeth and antibiotic treatment does nothing for dead bacteria. In some cases, a visually minimally invasive periodontal surgery is necessary to remove the rest of the etiology of gum disease.
